What is LCOH MyChart?

LCOH MyChart is a free, safe and secure online application that allows you and any other Lindner Center of HOPE/Lindner Center Professional Associates patient* to access, manage and receive your personal health information from a mobile device, tablet and computer at any time. *Available for Lindner Center of HOPE/Lindner Center Professional Associates patients ages 18+. With LCOH MyChart, you can use the Internet to:

    Connect with your healthcare providers online
  • Meet with providers via video visits.
  • Request prescription renewals.

  • Access your medical information online
  • Receive test results.
  • Review your medications, immunizations and allergies
  • Receive preventative care reminders, health education information and discharge instructions.

  • Manage your appointments
  • View details of your past and upcoming appointments.
  • Update your information prior to appointments.

  • Access your family's medical information
  • Parents or legal guardians of children and dependent adults may request proxy access to view portions of their loved one’s medical records.

How do I register for LCOH MyChart?


Register online
  • Click here to sign up.
  • Click the "sign up now" button.
  • -Option 1: Enter the 10-digit activation code you received at your last provider’s appointment and your personal information as prompted.
  • - Option 2: No activation code? No problem. As long as you’ve received care at a LCOH facility, you can simply click the “sign up online” button and enter your personal information as prompted.
  • Set up a personal username and password.

How do I log into LCOH MyChart?

When can I see my test results in LCOH MyChart?

Lindner Center of HOPE and Lindner Center of HOPE Professional Associates immediately releases the results of most labs and tests to patient’s LCOH MyChart accounts. This means that you may receive some test results before your provider has reviewed them. Your test results could include sensitive information. If you are not comfortable seeing this information before you have discussed your results with your provider, you can wait to view them until you speak with your provider. When your provider receives the results of your test(s), they will be able to provide you with necessary information about the meaning of the results.

Will my provider contact me about my test results?

Your provider may contact you about your test results in any number of ways. For example, your provider could add a comment to your test results or send you a secure message within your LCOH MyChart account. They may also recommend that you make a follow-up appointment, provide you with additional instructions, or ask you to call the office to discuss your test results. If you have questions, you are always welcome to contact your provider’s office.

Where can I find information about my visits?

Details about your appointments and visits can be found in your LCOH MyChart account.

  • Who else has access to this information? View details about the LCOH MyChart privacy policy and security information. Parents or legal guardians of children and dependent adults may request proxy access to view their dependent’s medical records. If you are an adult and have consented to giving someone proxy access to your personal health records, they may be able to view information. It is important to keep your personal LCOH MyChart login and password information private and remembering to select “log out” when you’re done reviewing your information.
  • Is it possible for me to see my entire medical record? All patients have a right to request their medical record from the Lindner Center of HOPE’s Health Information Management Department. Learn more about how you can make a request or call 513-536-0205.

If some of my health information on LCOH MyChart is not correct, what should I do?

If you feel that an item in your LCOH MyChart account is not accurate, please contact the Health Information Department at Lindner Center of HOPE. You can also ask your provider to correct any inaccurate information. Your health information is reviewed and updated in your electronic medical record during each visit.

Can I access my medical record using an application or website other than LCOH MyChart?

You may be able to access your medical record using your LCOH MyChart (MyChart) login information through third-party applications available on the Apple App Store, Google Play Store or on websites that support the Fast Healthcare Interoperability Resources (FHIR) communication standard. Outside of the LCOH MyChart (MyChart) website/application, LCOH MyChart does not operate, control, or endorse any other information, products, or services provided by third-party medical records applications or websites. Use of other sites is strictly at an individual's own risk. Individuals are responsible for viewing and abiding by the terms and conditions of use and the privacy statements of the other websites and applications.

If I agree to share my medical record with a third-party application or website, how long can they access my information for?

If you agree to the terms and conditions of a third-party application or website that allows them to access your personal medical information through your LCOH MyChart (MyChart) account, you will be granting them access to your information for up to one (1) hour each time you log in to the application/website. You can shorten the one hour access window by manually revoking access using the "Manage My Linked Apps and Devices" settings found on your LCOH MyChart (MyChart) account.

Can I view a family member's health record in LCOH MyChart?

Yes, you can. This is called Proxy access and allows a patient to log into their personal LCOH MyChart account, and then connect to information regarding their family member. Complete a Proxy Consent Form here. You can mail the form to the address listed or return it to one of our medical facilities to request access to this convenient service.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own LCOH MyChart account.

If I have a video visit scheduled, how do I attend it?

If your provider has scheduled a video visit with you, we ask that you please review the LCOH MyChart Video Visit Guide for Patients. This includes complete information and step-by-step instructions on how you attend your video visit. Please view the patient guide at least 48 hours before your scheduled video visit appointment to make sure your mobile device meets the criteria to attend a video visit. Video visits are currently available on a mobile device—not a computer.

How is LCOH MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal ID's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, LCOH MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with LCOH MyChart. Unlike conventional e-mail, all LCOH MyChart messaging is done while you are securely logged on to our website.

What is your Privacy Policy?

LCOH MyChart is owned and operated by Lindner Center of HOPE and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by Lindner Center of HOPE/Lindner Center of HOPE Professional Associates.

I was logged out of LCOH MyChart, what happened?

We aim to protect your privacy and security of your information. While logged into LCOH MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of LCOH MyChart. We recommend that you log out of your LCOH MyChart session if you need to leave your computer for even a short period of time.

My activation code does not work, what should I do?

For your security, your activation code expires after 60 days and is no longer valid after the first time you use it. You can sign up for LCOH MyChart without an activation code. Is my activation code my user ID?

No, your activation code is not your LCOH MyChart ID or password. You will use this code only once to log into LCOH MyChart for the first time. (The code will expire after you have used it or after 60 days). When you log into LCOH MyChart the first time, you will then be asked to create your own unique LCOH MyChart ID and password. You can sign up for LCOH MyChart without an activation code. Simply click here to sign up for a new LCOH MyChart account.
